How to Complete the Required Forms
All of the forms below are pdf fillable documents. There are 3 ways you can complete them.
1. Download them & complete them on the computer. Save the document after filling it out and upload it below.
2. Print the documents or use the forms in your folder from the tour, fill them in by hand, scan them and upload them below.
3. Print the documents or use the forms in your folder and drop them off at the school.

2: If you're ready to complete the Tuition & Enrollment Agreement for the program in which you are enrolling, please include that in the upload:
The $125 Registration Fee can be paid by check to Birmingham Bloomfield Montessori or you can use Zelle with the email billingbbmont@gmail.com. The receipt will come from BMC Education, Inc.
